An inspiring and informative testimony from Africa. It would be interesting to know a little about how these patient-directed models were brought about. Did MSF begin doing this in response to community and patient activist groups applying pressure for inclusion in service delivery systems, or did MSF make this decision strictly from within, based on, for example, some assessment of optimal service delivery?
